How to order:

• By Mail: Print the request form (pdf, which requires adobe reader), complete all requested information, and sign the form.

• By Email: If you have the ability to scan your signature, you may email your request to raileyl@evangel.edu
• By Fax: Print the request form (pdf), complete all requested information, and sign the transcript request form. Fax to: 417-865-9599
• In person: A request form can be filled out in the Records Office.

Payment Options:

• By Mail: Enclose Check or Money Order made payable to Evangel University or include credit/debit card information.
• By Fax: Credit/Debit Card (Information needed: Credit card type, number, name on the card, and expiration date).
• In Person: Check, money order, cash, or credit/debit card.

Unofficial Transcripts

An unofficial transcript may be obtained by printing the request form (pdf), completing all requested information, and signing the form. It may be faxed or mailed. Payment is not required.

Please note: We do not email or fax official transcripts. If you would like your request to be sent overnight, the fee is $10. Delivery is subject to service availability.
