Verification Forms

• Verification Form for Dependent Student
  These forms, along with student and parent federal income tax returns must be completed, signed and returned to our office before any aid will be awarded or disbursed.

• Verification of Employer Reimbursement/Advancement (pdf)
  As a Degree Completion (undergraduate) or Graduate student, the Student Financial Services Office is requesting that you inform us if you expect to receive tuition reimbursement / advancement from your employer.

  For the dependents of nationally appointed Assemblies of God US Missionaries. The General Council of the Assemblies of God must certify this form annually.
