This form is to be completed by the person filing the complaint within 10 days of the grievance if an informal resolution, as outlined in the Student Handbook or Academic Catalog, is unsuccessful. This form initiates the formal complaint procedure as outlined in the Student Handbook. The completed form will be submitted to the Vice President of Student Development.
